Application for Child Support Enforcement Services

The Family Support Division’s Child Support Enforcement (CSE) program provides a variety of services, which are described in the brochures “What’s This About Child Support?” and “Does Your Child Have A Legal Father?” Please consider reviewing these brochures to find the CSE service right for you.

To apply for CSE services, you must print and complete the application forms, then mail the forms to your nearest CSE county office.

The CSE application forms (PDF, 7 pages) are:

  • Your Rights and Responsibilities as a Recipient of Child Support Services — Informational document to be read and retained for your reference.
  • Notice to Non-TANF Applicants: Domestic Violence Services — If you have an issue with domestic violence, CSE may have information that will help you.
  • Application for Child Support Services — Four-page document allows you to select the specific CSE services you need and provide vital information that CSE needs to assist you.

When you have completed the CSE application forms and mailed them to your nearest CSE office, CSE will use the information provided to initiate a case. You will be sent a letter when the case is opened, providing you with contact information and a case number. Please keep this notice available for future reference.
